Support at Home funding
Your arrangements are confirmed before support begins
Social support must be part of your approved support plan where government funding is used. Support is subject to available funding, contributions, agreed arrangements and worker availability.
Working together
Fambulcare and Trilogy Care
Fambulcare delivers home and community support services through an associated-provider arrangement with Trilogy Care, where applicable. Trilogy Care is the registered provider responsible for its relevant Support at Home funding and provider obligations.
Fambulcare focuses on delivering personalised services and supporting clients with their day-to-day care needs.
Services are subject to approved care arrangements, available funding and applicable agreements.
