Understanding Support at Home
Support at Home is Australia's reformed aged care program, effective from 1 November 2025. It replaced the Home Care Packages Program and Short-Term Restorative Care Programme.
The program provides government-funded, coordinated care and services to help older Australians remain living independently at home. It's designed to be more flexible, person-centred, and responsive to individual needs.

What Support at Home covers
Your Support at Home funding can be used to access a wide range of in-home services. Fambulcare can discuss available options and the next appropriate step for your funding arrangement.

How to access Support at Home
Getting started with Support at Home is straightforward. Here's what to expect.
Contact My Aged Care
Call My Aged Care on 1800 200 422 or visit myagedcare.gov.au to register and request an aged care assessment.
Undergo a Single Assessment
An assessor will visit you at home to determine your care needs. The new Single Assessment System (from 1 July 2024) simplifies this process.
Receive Your Approval
Once approved, you'll receive confirmation of your eligibility for Support at Home and be placed on the national queue.
Connect with Fambulcare
When your support becomes available, Fambulcare can help you connect with a suitable registered provider and broker the right supports on your behalf. We'll work with you to build a personalised care plan.
Transitioning to Support at Home
If you were receiving Home Care Packages services before 1 November 2025, your care has automatically transitioned to the new Support at Home program. Your provider will have guided you through this transition.
