Associated-provider care coordination
Practical care coordination with a clear point of contact.
Where care coordination is allocated under the applicable arrangement, Fambulcare can help clients and families organise approved practical support and keep communication clear.
Working together
Fambulcare and Trilogy Care
Fambulcare delivers home and community support services through an associated-provider arrangement with Trilogy Care, where applicable. Trilogy Care is the registered provider responsible for its relevant Support at Home funding and provider obligations.
Fambulcare focuses on delivering personalised services and supporting clients with their day-to-day care needs.
Services are subject to approved care arrangements, available funding and applicable agreements.
Care coordination
Making an approved care arrangement easier to manage.
Coordinating care can involve many conversations, services, appointments and decisions. Where the activity is allocated under the applicable arrangement, Fambulcare provides a consistent point of contact to help eligible clients understand their approved services and organise practical support.
We take time to understand routines, preferences, goals and concerns. With consent, we work with the client, chosen supporters, workers and Trilogy Care to help services stay organised and responsive.
What we can help with
Practical coordination, where it is part of the agreed scope.
Depending on the approved care plan and arrangement, care coordination may include:
- Discussing goals, routines and personal preferences
- Helping organise approved services and appointments
- Supporting communication with family or representatives with consent
- Following up scheduling concerns or service issues
- Supporting communication between clients, workers and Trilogy Care
- Identifying changes that may need a care-plan review
Care coordination does not replace clinical assessment, emergency assistance or decisions that must be made by the registered provider or an appropriately qualified health professional.
